This video shows how your customers will verify their email address, read your terms and submit their response through your Smudge Agreement Form.

Instructions
1 - Enter your email address
Once you press the 'Send Code' button, you will be sent an email to this address containing your 6 number verification code.
2 - Enter verification code
Once your code has been sent, the form will change to show a new field where you should enter the verification code that you have received.
Enter this code then click the 'Verify' button.
(If you have not received this code, please check your email's Junk folder)
3 - Read the terms shown
Once you have successfully verified your email address, you will be shown the terms entered by the administrator of the form.
You must read, understand and agree to the terms that you are shown. You can then enter your full name in the field provided, click the 'I Agree' tickbox and then press the 'Submit' button.
4 - Form Submitted
Once your response to this form has been submitted successfully, you will be shown a 'Continue' button that will take you to a website entered by the administrator of this form and details of your agreement to the form will be saved to their console.
